Banner Life Insurance provides a level term product called “OPTerm”, for customers who want larger amounts of term insurance coverage at competitive rates.

This product is available to anyone between the ages of 18 and 80.

Maximum Amount	None
Underwriting/Rates	Underwriting and rates are determined by applicant’s age, gender, underwriting health classification, nicotine usage, policy plan, and coverage amounts.
Discounts	Volume Discount Levels: 100,000 – 249,999 250,000 – 999,999 1,000,000 – over
Exclusions	Suicide during first two years of policy.
Beneficiary	Applicant can appoint any beneficiary. Note: GreenStone should not be named as the policy beneficiary.
Plan Options	10, 15, 20, 30-year terms. Note: Plans offered can change frequently.
Waiver of Premium (small additional premium)	Waiver of Premium - Premium is waived if insured is totally disabled. Coverage ceases at age 59.
